Referral Policy

To Refer A Client To The Animal Medical Center

Your client may call the AMC Appointment Desk at (212) 838-7053 seven days a week or, for personalized service, you may call our Referral Office directly at (212) 329-8758 between 9 a.m. and 5 p.m. (closed Sundays).

1. As the Referring Veterinarian, you should fill out the AMC Patient Referral Form, listing the medical problems, relevant treatments and the reason for referral.
2. Attach relevant diagnostic/lab test results and medical record copies.
3. Be sure to include radiographs (labeled with your name and return address) and a copy of a current rabies vaccination certificate.
4. Return all forms and records to our Referral Office Fax Line at (212) 832-5417.

To Make An Emergency Referral

Our Priority Emergency/Critical Care Hotline at (212) 329-8616 is a special service that The Animal Medical Center offers to our Referring Veterinarians. With the Hotline, a Criticalist is always available to speak to you directly, noon-midnight, 7 days a week.

In an emergency, send your client directly to The Animal Medical Center, then fax your client's information to Medical Records at (212) 752-2592.
