Animal Medical Center Gift Card Terms & Conditions

Your Animal Medical Center Gift Card will be honored for medical services, medication or pet food at The Animal Medical Center. This Card is not redeemable for cash, except to the extent required by law, and cannot be replaced if lost or stolen. Purchases will be deducted from your Card until the value reaches zero. By accepting and using this Gift Card you agree to the terms and conditions now in effect or as may be required by law. The Animal Medical Center reserves the right to change the terms and conditions of its Gift Card program.

1. The Animal Medical Center Gift Card is intended for use as a gift to an individual. Resale of this Card or use for unauthorized advertising, marketing, sweepstakes or other promotional purposes is strictly prohibited. The
Animal Medical Center reserves the right, in its sole discretion, to refuse to honor any gift card that is used in a manner inconsistent with these terms and conditions, including without limitation any fraudulent use. If The
Animal Medical Center suspects misuse of the Card, it may be suspended or terminated.

2. The Animal Medical Center Gift Cards are issued and sold by The Animal Medical Center. By purchasing, accepting, retaining or using this Gift Card, you agree that the laws of the state of New York, without regard to
principles of conflicts of laws, will govern these Gift Card terms and conditions.

3. The Animal Medical Center Gift Cards may be used for legitimate purchases of medical services, medication and pet food through The Animal Medical Center and not for the purpose of obtaining cash from The Animal
Medical Center. You may not return food or medications obtained with an Animal Medical Center Gift Card for a cash refund.

4. Certificate cannot be reproduced; Any attempted reproduction will render the certificate void. Expiration date of this certificate is 5 years from
date of purchase.
